Our privacy policy covers the collection of personal information in our office and through this website.  This privacy policy is in effect January 1, 2015 and may be updated or changed at any time without notice and without immediate publication on this site.


Personal information includes name, date of birth, gender, ethnicity, phone number, email addresses, mailing address, medical history such as medical conditions and medications being used, occupation, activities, and family medical history.  You agree to this privacy policy and and that this information is collected with your consent as a user of this website, a patient of our office or as a retail customer of the business.


Our primary method of communication with you is email.  If we are unable to reach a patient by email we will use the telephone number as a secondary method .  If at anytime you wish for us to cease communications with you either by email or telephone please notify us in writing or email to have your personal information removed from our database.


There are several parties who may have access to your personal information.  Physicians and optometrists who are affiliated and/or practice at king west eye care will collect and review this data as it pertains to the eye health examination, selection of treatment options, ophthalmic products, and regulatory requirements of licensing bodies.  Ancillary staff employees and volunteers at king west eye care will have access to and use this data for the purposes of office administration, data entry, recalling appointments, confirming appointments, electronic health records, notifications, promotions, billing, pre-examination testing, and referrals to other health  care professionals.  From time to time a medical condition will require us to refer a patient to another health care practitioner for further care.  In these cases personal information will be transferred to another party for management under another privacy policy of which we cannot be responsible for.  Personal information may also be directly shared with other health care practitioners in the form of consult notes in the ongoing care of a mutual patient.


Any requests by a third party for your personal information   must be made in writing with your written consent to our information officer Dr. David Chu O.D.  Only after this request has been reviewed by our information officer will this information be released.   This review process may take up to 14 days when the information officer is not available.  We maintain the reasonable security and confidentiality of personal information by following this privacy policy.  Any information transmitted by email is not secure and is not recommended for the transmittal of personal information.  Any e-commerce is handled by a commercial third party.  We use td merchant services, moneris solutions, and paypal incorporated for secure 128 bit ssl credit card processing.
